  1. Fill sample bottle to no more than ¾ full
  2. Install cap until completely tight
  3. Fill out sample information form completely
  4. Place bottle in Ziploc bag, remove air and seal
  5. Place bottle/bag and sample information from inside mailer bag
  6. Peel off sealed liner, fold over and seal
  7. Drop off at any USPS pickup location
